NOTE: Anyone making false declarations can be prosecuted under the Infectious Diseases Act.

Please complete this travel and health declaration on the day of your visit and show to our staff before clicking "Submit".

You WILL NOT BE ALLOWED ENTRY if you are currently issued a Medical Certificate (MC), or under Stay Home Notice (SHN), Home Recovery Programme (HRP), Isolation Order (IO), Self-isolation OR awaiting a COVID-19 swab result.

My child is NOT serving an IO, SHN or awaiting a COVID-19 swab test result.

My child is NOT experiencing symptoms of acute respiratory illness (e.g. cough, shortness of breath or difficulty breathing, fever, new loss of taste or smell, sore throat, muscle pain or body aches, headache, or chills) or other respiratory illnesses.

My child is NOT issued with Health Risk Warning by MOH or if issued with HRW, has a valid 24-hour negative ART result before attending the programme.

Anyone making false declarations can be prosecuted under the Infectious Diseases Act. The Infectious Diseases Act is the principal piece of legislation that deals with the prevention and control of infectious diseases in Singapore. The information you provide is important in managing the risk of COVID-19 transmission. A person who knows, or has reason to suspect, that he is a case or carrier or contact of COVID-19 shall act in a responsible manner to not expose other persons to the risk of infection.
